Your certificate for the CME hours earned at the MPC will be emailed by April 16, 2010. It will be sent as a link to the email you provided when you registered for the Conference. Please watch for this email so that you can print your certificate for your records. If you did not provide an email or you wish to have your certificate sent to a different email please download the Certificate Request Form and fax it back to 312-427-5813 or email it to assistant@midwestpodconf.org. You may also mail it to: Midwest Podiatry Conference, 122 S. Michigan Ave., Suite 1441, Chicago, IL 60603. There will be a $5 charge when you request a 2010 certificate to be mailed instead of emailed.

Requests for Certificates Prior to 2010:

If you need a Certificate from 2009 or earlier, download the Previous MPC Certificate Request Form and send it to the MPC office as noted above. There will be a $10 charge to fax, mail, or e-mail a certificate from a previous year.
